Programme Manager - GP/Primary Care Networks
Practicus is working with a PCT cluster in the South East, supporting them with Transformation in Primary Care.
The cluster and its Clinical Commissioning Groups are embarking on a programme of work to set up a number of clinical care networks across their localities in order to support their out of hospital/care closer to home strategies. The care networks (aka GP networks) will provide joined up primary care-focused care to patients within specific care areas, Diabetes for example.
We are seeking a Programme Manager to work from the approved business case to develop the programme plan, manage it through to completion, including recruitment of the programme delivery team.
The ideal candidate will fully understand the current primary care drivers and initiatives, and will have extensive experience of CCG / GP engagement. Programme management of complex NHS transformations is essential, as is managing and leading project and commissioning teams.
