Practicus is working with a PCT cluster in the South East, supporting them with Transformation in Primary Care.
The cluster and its Clinical Commissioning Groups are embarking on a programme of work to set up a number of clinical care networks across their localities in order to support their out of hospital/care closer to home strategies. The care networks (aka GP networks) will provide joined up primary care-focused care to patients within specific care areas, Diabetes for example.
We are seeking a Locality Manager to work to a Programme Manager to deliver this transformation change. This will involve very close liaison with the individual GP practices within the network, supporting them with the establishment of the networks including governance, patient data, practice business planning, information sharing etc. This will be dedicated support throughout the planning and mobilisation phases.
The ideal candidate will fully understand the current primary care drivers and initiatives, and will have extensive experience of CCG / GP engagement. NHS primary care project management / locality management / commissioning experience is essential. You should have worked at 8c level ideally.
Our client requires someone for the duration of the programme (12mths) and we are able to pay £250-300pd.
