About the role

We are looking for a brilliant individual to lead the design of a profitable, high quality and distinctive Intermediate Care health service offer for Home Group.

Working to our new Head of Clinical Practice this is a rare and exciting opportunity to lead the implementation planning of our new service which promotes independence and supports customers with complex needs including intermediate care, reablement, dementia and/or frailty.  In this role you will ensure transition of this new service into operational delivery and focus on the development of services as set out in the NHS 5 year Forward View.

Who we’re looking for

We are looking for someone with strong experience of leading complex care services either in the NHS or public services setting.  You will be an excellent negotiator and influencer with a great knowledge of reablement best practice including digital solutions to support independence. You will also have experience of delivering integrated services to promote recovery and independence and of clinical/alternative interventions to support complex care customers. 

Together with a collaborative and proactive approach you will be committed to the delivery of new business growth in reablement services in line with Home Group’s new strategy.
