Our client, a CCG In the South East are looking to recruit an interim Project Manager for long term condition for an initial period of 6 months.

The Post Holder will lead on the implementation of an ambitious transformation portfolio with a focus on long-term conditions in primary care and out of hospital settings. The aim is to develop high quality services for people with long-term conditions with an emphasis on prevention, self-management, risk stratification, collaborative care planning as well as promoting a multi-morbidity approach. This will deliver reduced variation in service and patient outcomes, address health inequalities and empower patients.

The role will require building effective relationships with a wide range of stakeholders and partners including general practitioners, CCG clinical leads, other health and care commissioners, and patients and public representatives.

The post holder must have:

  • Excellent communication and negotiating skills, ability to engage and work with a wide range of partners, providers and patient representatives
  • Experience of commissioning long-term conditions services
  • Good understanding of the Primary care landscape and general practice
  • Experience of leading and delivering integrated services programmes of work
