Interim Integrated Care Programme Manager
A CCG requires an Integrated Care Programme Manager to lead on a Community Independence Service as part of a joint programme with the Local Authority.
As the Programme Manager for Integrated Care you will be responsible a complex programme of change and the further improvement of their Community Independence Service. This requires an understanding of the integrated health and social care agenda as well as the ability to work across complex partnerships and commissioning to join up strategic direction with operational improvements, working hand in hand with the lead provider and provider network.
You will need to be able to demonstrate experience managing commissioning programmes within an integrated setting, working across partnerships and ideally have worked on a Community rehabilitation service.
