Positions immediately available for independent nurses to work in reputable NHS trust as CHC (continuing healthcare) Nurse assessors.

This role allows you to make a real difference in the community as you are able to visit those who have been identified as in need of a full CHC assessment. By completing the decision support tool, you will be able to recommend the level and types of care people suffering with learning disabilities require.

Essential skills:

*To have a valid nursing pin.

*To be experienced in a community setting.

*to have conducted DST's previously.

*To be familiar with the process's involved in CHC.

*To have experience as part of a Multidisciplinary Team (MDT)

Desirable skills:

*To have reported into, or chaired panels.

*To have experience in the appeals process.

*Experience with dealing with high cost packages of care and complex cases.
