Job Summary

  • Completing DST in line with the NHS National Framework and Funded Nursing Care
  • Managing a defined caseload of CHC reviews
  • Following Continuing Healthcare Guidelines and that the services delivered are met to maximize patient safety by meeting their ongoing clinical needs
  • Making the best use of resources and ensure that services are clinically appropriate
  • To be responsible for a caseload which complex Continuing Healthcare patients and support the commissioning of bespoke packages and to provide ongoing monitoring and review for a specific group of patients  

Main Duties

  • Completing the Decision Support Tool (DST) and reviews of DSTs.
  • Ensuring that the completed DST which has been sign by the MDT is submitted in a timely manner to CCG for a decision on the individuals eligibility 
  • Work in conjunction with a range of professionals to ensure that all individuals and professionals involved in care of a service user are included when completing DST
  • Provide the organisation, where necessary, with expert advice on the National Framework for CHC and NHS FNC
  • Participate in Best Interest meetings or MDT meetings for services users
  • Prepare a range of reports on client, as appropriate, in accordance with policies and procedures
  • Ensure compliance with Safeguarding Policies and procedures 
  • Review and monitor implementation of placements / packages of care
  • To undertake other duties as fall within the scope and grade of the post including the completion of CHC assessments and reviews when required
