Job Details
Main Responsibilities:
- Successfully support the CHC senior management in the development of the CHC Service;
- Identify service gaps act as an expert in CHC;
- Develop and monitor clinical/practice standards for the service and contribute to the annual audit plan;
- CHC Specialist Services in investigating complaints and ensure there is learning from these;
- Involved in the approval of Fast Tracks and in brokering care packages;
- Responsible to provide value for money whilst ensuring quality of all care packages brokered;
- Develop an in-depth knowledge of the provider market in general and will develop working relationships with service providers;
- Knowledge and expertise to influence Contracting/Finance and CHC Quality strategy and developments
- Work in partnership with hospital discharge teams, hospices, community health teams and Social Care;
- Support brokerage practitioners to present cases to the CCGs Complex Case Resource Panels and, where appropriate, to present complex cases in person to the same panels;
- Monitor and ensure that approved protocols/guidelines used for Fast Tracks and brokerage are implemented to a high standard and to review and report on any issues;
- Act as patient advocate supporting vulnerable adults and where necessary identifying and report any situations of adult abuse;
- Communicate with patients and relatives regarding Fast Tracks and brokerage of care packages.
- Working knowledge of all aspects of CHC including Clinical, Brokerage, Contracting, Finance, Business Intelligence and Quality;
- Demonstrate tact & diplomacy, empathy & sensitivity;
- Excellent knowledge of CHC national and local policy;
- Knowledge of service improvement and understanding of Change Management;
- Driving performance by using complex data;
- Act as a leader whilst being a good team player;
- Experienced in Operational Management;
- Excellent problem solving, negotiation and advanced numeracy skills are key to this role;
- Current clinical qualification and professional registration (e.g. NMC, HCPC).
