Personal Health Budget Nurse

I am currently seeking a Personal Health Budget Nurse to support the team in the delivery of personal health budgets, the monitoring and evaluation of each PHB and all reporting and audit activity relating to PHBs.

Key Responsibilities and Duties

  • Deliver an effective Personal Health Budget (PHB) service, to ensure that patients/families are able to access PHBs where appropriate to do so.
  • Develop processes and policies required to implement, sustain and take forward PHBs for patients who are eligible to request a PHB.
  • To be responsible for the operational delivery of PHBs to the CCG population in accordance with national and local offers, ensuring appropriate safeguards on quality of care for individual patients.
  • To work with clients, their families and careers to explain all aspects of personal health budgets and to simplify the process of setting up and managing a personal health budget.
  • To act as a single point of contact for individuals and professionals across health and social care organisations to provide information and assistance in relation to all aspects of Integrated Personal Commissioning.
  • Use clinical knowledge and expertise to review individual PHB cases and agree funding and eligibility on behalf of the CCG within delegated financial limits and agreed protocols
  • To work collaboratively with the PHB administrator to actively involve clients, relatives/advocates to develop comprehensive support and budget plans.

With current Nursing and Midwifery Council registration, you will have significant experience of complex case management within a variety of clinical settings and will be able to demonstrate your ability to deliver projects and case management on time, within budget and with a clear evidence base.
