Job Details
We are currently working closely with an NHS Clinical Commissioning Group who are looking for an individual with experience with Personal Health Budgets to join their tam up until the end of March 2019 initially. This role will be working in Southampton.
You will be part of a small established Continuing Healthcare Team and you will be:
- Supporting the Team Leader in this work stream in the production of care and support plan clients who have or want a PHB
- This work will focus on at home clients so yes travel will be required, primarily within Southampton boundaries
You could be a Care Manager or a Non-Clinical individual from the Voluntary sector with an understanding of how to produce a care and support plan for PHB's.
