CHC Complex Case Manager

Are you a Registered Nurse with Continuing Healthcare experience who has worked with complex cases?

We are currently working with a Clinical Commissioning Group (CCG) who are looking for a CHC Nurse to join for an initial 3 month contract.

You will be responsible for holding a portfolio of complex cases working in line with the National Framework for Continuing Healthcare. You will be looking at complex Mental Health related cases (e.g. ABI, Huntingtons)

You will also:

• Complete and review Client Needs and Risk Assessments in line with CQC guidance
• Develop, review and implement Service User Personal Care Plans in line with CQC guidance.
• Manage client care packages, monitor client care and well being
• Liaise with Social Services and attend case conferences/ MDT meetings as required
• Attend and contribute to Care Service Manager meetings and other meetings as required

This role will be covering the Cambridgeshire area including Cambridge, Huntingdons or Peterborough. A car is essential for this role.
