NHS Project Transformation Manager - Community Services

Practicus are partnering with a London based NHS Trust who are urgently seeking an Interim Project Manager / Transformation Lead for the Single point of access programme for Community Services

A London based NHS Trust require an Interim Project Transformation Manager to deliver their Single Point of Access (SPA) Programme across their whole spectrum of community services within Planned Care, Un-planned Care and Mental Health. Your role will be to work across a wide range of Stakeholders within the Community providers and CCG to plan and implement service changes and transition them to new models of care taking into account the clinical pathways, operations, workforce and contract obligations. We are looking for someone that has exceptional stakeholder engagement skills coupled with a breadth of knowledge across a wide spectrum of clinical pathways. We are also looking for someone that is highly organised with strong project management skills with a passion for delivery.

Essential:
* Strong Service Transformation Delivery Skills
* Project Management experience
* Broad knowledge of Clinical pathways
* Knowledge of Community services would be highly desirable
* Excellent engagement skills, able to work across organisational boundaries
* Any experience of delivering a Single Point of Access programme would be desirable but not essential

RATE: £300per day (Inside IR35) or £350 per day 4 days a week
START: ASAP
