Primary Care Delivery Manager
Practicus are partnering with a London based NHS Organisation who are seeking a Interim Primary Care Manager.
The interim will play a key part in developing, supporting practices deliver transformational change in the way they operate and deliver care, changing the way healthcare is delivered in the region.
You will be instrumental in supporting localities and be a key link between localities, practices, the CCG and GP Fed. Lead on the practice resilience programme and will contribute to the development and monitoring of local incentive schemes, enhanced services and QIPP schemes that practices participate in.
As well as good project management skills, you will need to have excellent partnership and joint working skills, ensuring that all service developments are patient-centred and clinically-led, leading to sustained improvements in health, in quality of care, and in more effective use of resources.
The role will focus on improving system performance and transforming care by understanding and developing patient pathways and flows between primary, community and acute care, working with other colleagues to ensure that care is centred on patients and reduces duplication.
We have strong relationships with our practices so engagement and rapport is key.
- Good understanding of CCGs and the strategic context of primary care and general practice in particular
- Experience of working with GPs
- Engagement / building relationships
RATE: £300.00 to £325.00 per day (INSIDE IR35 - Part Time (3 days a week)
START: 4th March 2019
DURATIONS: 3 Months initially
