Job Details

A large healthcare organisation is looking to appoint an CHC Nurse Manager. Responsible for implementing National Policy and the framework for NHS Continuing Healthcare.

Main Responsibilities:
  • Support the CHC Team Leader in developing and implementing strategies for NHS Continuing Healthcare and fast track reviews in line with policies, including facilitating the change;
  • Work in collaboration with the Patient Advisory Liaison Service and other relevant organisations;
  • Implement systems and processes for Continuing Healthcare, while assisting in continual monitoring and analysis information against quality standards;
  • Maintain effective communication on issues in complex situations arising from the application of Continuing Healthcare criteria and the outcomes, including a range of inter-organisational professionals including GPs and Health Professionals, patients and families and providers as appropriate;
  • Make recommendations regarding eligibility to the Continuing Healthcare Senior Management for discussion at clinical panel meetings and prepare and manage appeal cases for current appeals and restitution cases for independent review;
  • Develop appropriate methods which facilitate the development of effective working relationships and involvement of key stakeholders including health professionals, patients and the public when making decisions regarding eligibility;
  • Maintain an awareness of legislation and changes to policy guidance affecting NHS Continuing Healthcare and Fast Track process;
  • Commission individual care packages for end of life care patients and their families, that are closely tailored to meet specific individual needs.
A successful candidate will have:
  • Specialist knowledge/training in management of the fast track pathway/and CHC framework and tools;
  • Experience of successfully managing budgets;
  • Improvement in services whilst achieving professional and personal credibility;
  • Highly developed written communication skills including report writing skills;
  • Nurse/NMC Registration, evidence of continuing professional and managerial development.
