Job Details
- Working in the community with Children at home assessing packages of care
- To meet caseload families 2 - 3 monthly to receive clinical updates and information, to attend TAC, CIN and ECHP meetings for our caseload, to formulate a positive working relationship with CCG nominated agencies providing the care for the caseload.
- 12 Case who require CHC - All 12 are due assessments.
- To clinically assess new referrals using the continuing care framework in place applying and following local policy. 2-3 referrals a month.
- To have knowledge of the continuing care framework to implement this as appropriate for our caseload.
- Support from the team and yourself, working
- All MDT work to be completed
