ResourceBank Healthcare are currently engaged by a large multi-speciality community provider in the Midlands who are in the process of establishing a PMO. There are several Programme Management requirements one of which is a Programme Manager for Clinical Transformation.

The contract will initially be for a 12-week period and the successful applicant will be responsible for two large and complex Clinical Transformation Programmes on behalf of the MCP.

The focus of the role will be two-fold - to lead on the establishment, development and mobilisation of a 24-hour Single Point of Access Hub for the MCP partners and to develop the integrated care network (ICN) model on behalf of the MCP too.

Main tasks & responsibilities:

  • You will have extensive knowledge and experience of ICN models and how they bring together health and care services to work in a more joined up way to provide care for patients;
  • You will have work closely with GP Practices and primary care providers to establish and develop ICNs across the MCP;
  • You will have need to work closely with Clinicians and will have experience managing successful Clinical Transformation Programmes for new and innovative models of care;
  • You will have a track record of working effectively with multi-disciplinary teams across a system;
  • You will have knowledge and understanding SPOA and understand the most effective ways to establish an SPOA service within Community provision;
  • A clinical background is not essential but is preferred;
  • You will ideally have experience in both a provider and commissioning organisation previously;
