Job Details
We are currently recruiting for a CHC Nurse Assessor in the North East to join a flourishing and diverse team for an initial 3 month contract.
The perfect candidate will be experienced in the following:
- CHC Assessments
- A registered General Nurse (RGN)
- Experience working in CCG/ CSU as a nurse assessor
- Funding Nursing Care
- DST's in line with the National Framework
- MDT's
- Good Communicator and team player
- Must have worked in Continuing Health care in the last 6 months at least
- Strong understanding and application of CHC Framework
- Live case assessment experience in CHC
- Excellent verbal and written skills
