Job Details

A large healthcare organisation is looking to appoint an experienced CHC Nurse to provide short term cover to the service.

Main Responsibilities:
  • Work under the guidance and support of the Clinical Team Leads to implement the National Service Framework for NHS Continuing Healthcare and NHS funded Nursing Care for residents in the location;
  • Promote best practice and provide professional advice to nursing homes and other providers, promoting good relationships and liaising with colleagues from other disciplines both internally and externally to the CCG;
  • Undertake statutory reviews and ensure that client’s needs have been met and progressed through the appropriate care pathway within agreed timeframes;
  • Ensure all reviews, reassessments include appropriate reports and evidence to support the level of care need identified within the Health Needs Checklist / Decision Support Tool / Needs Portrayal process;
  • Work with provider services to assist in the development of competent assessors to undertake assessments and reviews;
  • Provide contemporaneous records on all clients and reports as necessary adhering to NMC standards for record keeping;
  • Responsible for collating evidence and information for the Decision Support Tool and liaising with the multidisciplinary team for a team recommendation to be made for eligibility for Continuing Healthcare funding;
  • Manage day to day communication with other team members within the continuing healthcare team;
  • Coordinate and facilitate ‘fast track’ patients in line with operational policy and within agreed timescales. Ensuring the patients that are eligible receives a package of care or placement appropriate to their needs ensuring best value;
  • Responsible for recording client contacts and communications via file notes to be dated and signed and kept in master client file;
  • Work within the National Framework for Continuing Healthcare and NHS-Funded Nursing care.
A successful candidate will have:
  • Must have a current registration with the NMC as an RGN;
  • Ability to conduct nursing needs assessments, analyse the information collected in order to make an informed recommendation as to eligibility for Continuing Healthcare or for Funded Nursing Care;
  • A high level of professionalism, passion and dedication to deliver timely standards of work under challenging deadlines, with a solid capacity for attention to detail and a high degree of accuracy.
