Job Details

A large healthcare organisation is looking to appoint an experienced Nurse Assessor for Children’s and Young People’s Continuing Care Service.

To work on an interim basis in the East of England for 3 days per week.

Main Responsibilities:
  • Responsible for collating evidence and information, liaising with the multidisciplinary team for a team recommendation to be made for eligibility for Continuing Healthcare funding;
  • Current knowledge of the CYP Continuing Care Framework;
  • Familiar with report writing in accordance with the requirements of the framework;
  • Promote best practice and provide professional advice, liaising with colleagues from other disciplines both internally and externally to the organisation;
  • Ensure all reviews, reassessments include appropriate reports and evidence to support the level of care need identified within the Health Needs Checklist / Decision Support Tool / Needs Portrayal process;
  • Manage day to day communication with other team members within the continuing healthcare team;
  • Coordinate and facilitate ‘fast track’ patients in line with operational policy and within agreed timescales;
  • Ensure patients that are eligible receive a package of care or placement appropriate to their needs ensuring best value.
A successful candidate will have:
  • Must have a current registration with the NMC as an RNLD;
  • Must hold experience in the assessment of CYP care packages;
  • Car driver essential;
  • A high level of professionalism, passion and dedication to deliver timely standards of work under challenging deadlines, with a solid capacity for attention to detail and a high degree of accuracy.
