Job Details

An exciting new opportunity has arisen to work for a large healthcare organisation who is looking to appoint a CHC Placements Manager (Children’s).

The post holder will support the Deputy Director of CHC & Planning to ensure individual placements are managed effectively and to take the lead the delivery of the service to customers.

Main Responsibilities:
  • Ensure the placement request meets the clinical eligibility criteria and has clearly defined clinical outcomes, cost and length of time the treatment / placement will be required for;
  • Coordinate the review of individual placement arrangements providing advice and guidance to commissioners as appropriate;
  • Ensure safe storage of records in accordance with the data protection Act;
  • Manage and record all conversations with individuals and families where appropriate;
  • Provide support and advice to referrers including as applicable local care coordinators who are accountable for providing holistic case management for Mental Health patients;
  • Support the investigation and management of the serious incidents relating to individual placements;
  • Support the Assistant Head of Placements with governance and system management tasks, including dealing with complaints, enquiries and appeals;
  • Work with service providers to ensure that quality reviews to ensure all aspects of the placements are effectively monitored assuring delivery of joint action plans as required;
  • Act as a point of contact for supporting management of complex queries and working with;
  • Oversight and management of all aspects of the Team budgets.
A successful candidate will have:
  • Educated to degree level in relevant subject or equivalent level qualification or significant experience of working at a similar level;
  • Relevant professional qualification i.e. nursing, AHP, registered social worker or evidence of equivalent high-level experience of working at a similar level in specialist area;
  • Knowledge of NHS CHC and experience of completing eligibility assessments;
  • Experience of complex case Management.
