An exciting new opportunity has arisen to work for a healthcare organisation as a Quality & Patient Safety Facilitator on an interim basis for 3 months.

Assist in the planning and implementation of a strategic framework for continuous patient safety improvement, focusing on process and systems within and across the organisation.

Main Responsibilities:
  • Provide guidance in the monitoring of the organisation’s compliance with the Serious Incident Framework;
  • Ensure an accurate log is held of all trust-wide Serious Incidents, High Level Investigations and Never Events;
  • The post holder will be expected to have good knowledge in the Serious Incident (SI) process;
  • Requires supporting the Quality and Patient Safety with knowledge and experience in the field of patient safety, providing advice across the organisation on all matters related to patient safety;
  • Collaboratively work with staff of all grades across the organisation to ensure the SI process is managed as per the SI Framework;
  • Actively promote, develop and secure a patient safety culture by improving the knowledge and understanding of managers and staff through effective training and staff engagement programmes;
  • Ensure effective procedures are in place for the reporting and investigation of all serious incidents and significant events, including near misses;
  • Ensure that all serious incidents are reported onto STEIS in a timely and effective manner that complies with lead commissioner requirements thus reducing the likelihood of financial penalties being imposed on the Trust;
  • Ensure that Duty of Candour procedures are undertaken in accordance to organisation’s policy with respect to serious incidents and those incidents that do not met the serious incident criteria;
  • Ensure the submission of high-quality investigation reports, which include SMART action plans, to Trust commissioners within pre-defined timeframes.
A successful candidate will have:
  • Postgraduate degree/equivalent professional qualification or equivalent experience or willingness to undertake further training/education;
  • Experienced and trained within Datix and RCA;
  • Strong knowledge of the patient safety agenda and strong understanding of related regulations and framework;
  • Knowledge and understanding of clinical terminology;
  • Knowledge of the CAS system;
  • An understanding of the current issues relating to Governance within the NHS;
  • Broad experience of using information derived from large and complex data sets;
  • Ability to maintain effective working relationships in conflicting situations/scenarios.
