Job Details

An exciting opportunity has arisen within a large NHS organisation for an experienced Senior CHC Nurse Assessor – Band 7.

The post will require some on site working within the South East Coast area.

Main Responsibilities:
  • lead and manage a team of assessors who administer and undertake assessment of the patient’s eligibility for Continuing Healthcare and Funded Nursing Care (FNC) to plan and commission appropriate care packages;
  • Meet their needs, monitor that the care packages continue to meet their needs and review the planned care to ensure it provides a high standard of quality and safety;
  • Ensure that the assessor team keep accurate records and can demonstrate that funding recommendations are evidence based and made according to the criteria described within national CHC frameworks;
  • Contribute to the clinical management function of the CHC service, forming part of a senior duty roster to handle fast track applications, review checklist referrals, manage complex cases and sign off assessment ratifications;
  • The post holder will work with all stakeholders including patients and, where relevant, their families, to ensure that funding decisions are taken in a transparent and coherent manner;
  • Advocate for the vulnerable, challenging poor practice and discriminatory attitudes and behaviour relating to their care;
  • Act as a professional advisor / specialist resource to fellow practitioners and assessors for complex cases such as members of the community nursing teams, regarding issues relating to FNC & CHC assessments and reviews.
A successful candidate will have:
  • Must hold current registration to the NMC Register;
  • Experience of working within CHC and of applying the National Framework for Continuing Healthcare/community;
  • Experience and understanding of Change Management and leading projects;
  • Working knowledge of all aspects of CHC including Clinical, Brokerage, Contracting, Finance, Business Intelligence and Quality.
  • Effective communication and ability to build constructive relationships with a range of key stake holders including clinicians and patients.
