Job Details
Main Responsibilities:
- Providing cross system knowledge and experience to help families navigate services and access support;
- Providing challenge to services and system partners when necessary to ensure the best experience and outcomes for the CYP;
- Representing the needs of the young person and works with carers, services and key professionals across community and inpatient units;
- Involvement throughout inpatient treatment, working with others to facilitate timely discharge, well managed transition and implementation of CETR recommendations for community support;
- Ability to work as part of a dynamic team, supporting managers and staff across locality in delivering an effective service to ensure safe and effective care pathways are in place in specialised mental health care settings.
- Health and social care professional registration;
- Knowledge and experience of the Transforming Care agenda
- Ability to chair and support CETRs.
- Experience with LD /ASD clients
- Ensure there is a co-ordinated approach to care and discharge planning, which results in robust and effective care packages.
