Transforming Care Case Manager

Woodrow Mercer Healthcare have been retained by an NHS organisation to source an Interim Band 6 Transforming Care Practitioner a 3-month contract which will fall inside of IR35.

We are preferably looking to recruit a Registered Learning Disability Nurse to take on a case load management role. The post-holder will be confident in chairing CTR meetings and have experience in carrying out DST CHC assessments. The post-holder will be working remotely so no travel will be expected or required, equipment for this role will be couriered to yourself.

Essential Skills

• Qualified and registered Nurse
• Completed mandatory CHC National Framework Training
• Chaired Care, (education) and Treatment Review Meetings
• Experience carrying out DST CHC Assessments
• Must have worked within a CCG or CSU organisation

Desirable skills

• Registered Learning Disability Nurse
• Extensive knowledge of the CHC National framework
• Experience working as a Transforming Care Case Manager
