A large Healthcare organisation is looking to appoint a Patient Safety & SI Investigation Lead on an interim basis for 6 months.

The post will be working in the East of England on a band 8a, inside IR35. 4 days per week will be on site and 1-day remote working.

Provide professional expert advice and leadership in relation to clinical risk and clinical governance and to manage an effective clinical risk management department for the organisation.

Main Responsibilities:
  • Responsible and accountable for the delivery of elements of the risk management framework on a cross site basis including responsibility for managing the incident reporting system;
  • Responsible for ensuring incident and risk management reporting processes are embedded within the organisation;
  • Providing and evidencing analysis, trends and learning from incidents experienced within the organisation and for raising the profile of incident reporting and risk management and their role in delivering the quality and safety agenda;
  • Provide a focal point for incident reporting and encourage all staff to make use of the system and share resulting good practice and lessons learned;
  • Ensure systems and processes in place to support the timely reporting of “near misses”, adverse incidents and complaints across the organisation;
  • Support the investigation of serious incidents, at times as lead investigator and in a coaching role, ensuring a consistent and co-ordinated approach in line with the policies and national Serious Incident framework;
  • Ensure that all cases requiring statutory duty of candour are effectively tracked and that a monthly report is provided;
  • Advise, develop and implement a range of appropriate training programmes for staff across to ensure understanding and compliance with all relevant the Policies in relation to incident and risk management requirements.
A successful candidate will have:
  • First level Registered Nurse, Allied Health Professional or equivalent would be desirable;
  • Extensive expertise working within a healthcare organisation;
  • Experience of managing and leading a team;
  • Knowledge and understanding of clinical risk management and statutory requirements;
  • Experience in dealing with complex and/or sensitive information and unpredictable situations;
  • Extensive experience of investigating patient safety incidents with knowledge of a range of investigation tools;
  • A thorough understanding of the requirements of the Statutory Duty of Candour;
  • Ability to think clearly, facilitate and challenge appropriately and communicate effectively with staff of all disciplines and across all levels of the organisation;
  • Well-developed skills in report writing, including the ability to relay complex, sensitive or contentious information to the lay person and in difficult situations.
