Job Details

The  role will  take  a lead in supporting the system to drive forward transformation in both  the  design  and  delivery  of services  for CVD  and  neuro  rehab,  including ensuring value for money.

Main Responsibilities:
  • The role will include a central focus on health inequalities and prevention as part of planning and service development in relation to CVD and neuro rehab. In this regard the role will take a Population Health Management approach allowing for delivery against system level outcomes;
  • Responsible for the strategic approach for CVD and neuro rehab, including plans for delivery and transformation at place level;
  • Designed to build a combination of networks, subject matter expertise and technical skills to allow for robust patient services and care pathways in relation to CVD and neuro rehab;
  • Work closely with ICPs, secondary care providers, community service providers, GP practices and other health and social care providers/commissioners to support implementation of service change;
  • Ensure optimum use of resources, including financial and activity modelling and monitoring; central link to regional networks, thereby ensuring that the strategic commissioner is pursuing best practice and meeting relevant outcomes;
  • Provide and receive highly complex, sensitive and contentious information, presenting information to a wide range of stakeholders in a formal setting.
  • Commit to working and engaging constructively with internal and external stakeholders on a range of business sensitive issue;
A successful candidate will have:
  • Must have a working understanding of the relationship between the Department of Health, NHS England and individual provider and commissioning organisations;
  • Experience of working across complex networks, commissioning arrangements to deliver transformation and change;
  • Proven experience working within and on clinical pathways for Stroke (CVD) & Neuro Rehab.
