An exciting opportunity has arisen, supporting a large NHS organisation as a Nurse Assessor.

The post will be based in the East of England, initially on an interim basis for one month, with the possibility for an extension.

The post holder will be responsible for liaison and communication between the Hospital and appropriate community services, for informal and formal carers to facilitate a smooth, safe, and effective transfer from hospital to home and vice versa.

Main Responsibilities:
  • Develop and promote the safe and efficient transfer between acute and community services; Advise hospital and community nursing staff, and where appropriate, medical, and paramedical colleagues on aspects relating to safe and effective transfers of care provision;
  • Manage a caseload of complex patients under designated directorate, with the support of the Senior Nurse Assessor, being the first point of contact for ward staff, and able to identify the need for escalation to Senior where required;
  • Responsible for carrying out health assessments including Mental Capacity assessments with the support of Senior Nurse Assessor;
  • Directly assess patient’s nursing and social needs to plan for a safe and effective discharge;
  • Liaise directly with relatives to ensure good communication for safe discharge plans;
  • Responsible for ensuring there are no inappropriate delayed transfers of care within their designated directorate;
  • Liaise closely with the Clinical Site Manager to facilitate timely planned discharge and identify re admitted patients and to be competent in the understanding and use of tele-tracking;
  • Promote the policy for the use of the Discharge lounge, identifying delays for audit and planning purposes;
  • Encourage participation between the community services and hospital staff prior to discharge of complex cases;
  • Maintain and provide accurate records, reports, and statistical information as required;
  • Set realistic objectives and implement new initiatives in conjunction with other members of the multidisciplinary team for the continuing development of the service, and develop protocols;
  • Support hospital flow by providing accurate data reporting of discharge services which is fed into bed meetings;
  • Review all health-related cases prior to referral to the external partners;
  • Provide clinical and professional advice and guidance to staff, as required;
  • Supply a clear focus for clinical leadership and be responsible for ensuring the provision of a high standard of holistic, patient-centered care.
A successful candidate will have:
  • A healthcare professional registered with the NMC, and relevant Acute clinical experience;
  • 4 years post registration clinical experience, which must include a minimum of 3 years’ experience within an Acute Trust;
  • Demonstratable success in working across professions and services;
  • Highly developed communication skills, written, verbal and presentation, with the ability to give constructive feedback.
