Practicus are partnering with an NHS organisation who are urgently seeking Interim Band 8a Project Manager - Outpatients Transformation

The project manager is responsible for transforming the delivery of follow-up care for outpatient services through maximising the opportunities associated with patient initiated follow up and remote monitoring.

Scoping and implementing the work stream delivery model for personalised ongoing care in relation to outpatient services. Working with and developing key relationships with a range of stakeholders, including the regional team, local Trust, and system partners, to mobilise all relevant activity for personalising ongoing care in outpatient services

Essential:

  • Extensive experience in complex programmes in the NHS
  • Ability to manage and communicate risks to delivery and work with stakeholders to mitigate these
  • Experience in supporting front line services to adopt a change in service delivery processes and achieve measurable improvement, in a large-scale organisation or nationally.

RATE: Band 8a

START: November

INTERVIEWS: w/c 18th October

DURATION: 12 months
