A large NHS Trust is seeking a Deputy Director of Care Coordination within the East of England for a period of 6 months. 

Main Responsibilities:
  • To support the Trust to improve people’s outcomes and experience by supporting the system to navigate people to the services they need, at the right time, and in the right place;
  • To provide a referral hub for all system partners to access community physical and mental health and care services;
  • To lead on the development of the patient flow, access and Care Co-ordination programme in partnership with multiple system organisations and partners;
  • To support and motivate staff within the Care Co-ordination Centre and wider system, including clinicians, professionals to ensure the people are supported in meeting operational and programme requirements.
A successful candidate will have:
  • Current NMC or equivalent registration as an AHP/Social Worker;
  • Experience of leading and successful delivery of strategic transformation/change programmes;
  • In depth understanding of the processes associated with patient flow between acute and community;
  • knowledge of General Management, Financial Management, Business Planning, and Clinical/Professional Strategies acquired through training and experience.
