Liaison Practitioner Learning Disabilities

A specialist Acute Trust is seeking an Liaison Practitioner Learning Disabilities to provide support and advise in the care of patients with Learning Disabilities. This position will be for 3 months initially.

Main Responsibilities:

• To support the Clinical Nurse Specialist in Learning Disability in the delivery of services to people with learning disabilities;
• To ensure that people with learning disabilities are safe when using the hospital services and ensuring that the safeguarding adult’s procedures are followed;
• To work with the discharge facilitators to develop and maintain good practice guidance for the discharging of people with learning disabilities from the Trust, mapping links to community services;
• To implement core care planning for people with learning disabilities that can be used by hospital based staff;
• To participate in the development and training of hospital based staff to build capacity and knowledge in supporting people with a learning disability;
• To liaise with the clinical leads and members of the multi-disciplinary healthcare team to achieve a co-ordinated patient centred service.

A successful candidate will have:

• RNLD / OT/Physio or S&LT qualification with appropriate rotational post experience;
• experience in managing patients with Learning Disabilities;
• Have a clear understanding of the role of Liaison Practitioner within speciality;
• Understanding of clinical governance and risk assessment.
