A CCG is seeking an Interim Programme Manager with experience of discharge to assess / Home First for a minimum 6 months outside of IR35. You will need to have previous experience of change programmes in Health and Social Care and in particular you must be able to demonstrate with examples where you have previous experience in this area.  

The discharge to assess national programme aims to support local health and social care systems to reduce the time people spend in hospital, at the point that they no longer need acute care. Your role will be to work closely with the Local Authority to support the ICS system to ensure the local people in the locality wherever possible, should be supported to return to their home for assessment. Implementing a discharge to assess model, where going home is the default pathway, with alternative pathways for people who cannot go straight home

You will need to use your experience in this area to work with all relevant stakeholders and parties to put in place and support a change programme that enables the system to have an efficient system and process in place.

You must have Health and Social Care experience in discharge to assess / Home first initiatives and programmes to be considered.
