We are working with an NHS organisation who are seeking to appoint an Interim Deputy Director of Integrated Care Transformation. This role will be for 3 months and is based on site.

This is an interim role that falls under the Workforce Alliance Framework and should be expected to be remunerated between c£445 - c£540 per day, dependant on experience.

Key responsibilities will include:
  • Lead on the trust-wide integration strategy and agenda with community, social care and
  • primary care partners;
  • Focus on admission avoidance, community services (including LTCs and community hubs), and discharge pathways;
  • Day-to-day operational, as well as strategic, management and delivery of Discharge to Assess and related flow pathways across the organisation, working effectively across all Care Groups and with partners;
  • Accountable for delivery against admission avoidance and community hub/pathway aspirations and targets; supporting effective and timely organisational and system-wide patient pathways and flow;
  • Lead the delivery of the Discharge system programme as well as, community service and admission avoidance programmes and internal workstreams.
The ideal candidate will have:
  • Degree/Masters level qualification in a relevant area to this role or equivalent level of experience in a related field;
  • Extensive senior management experience of working in an Acute and/or Community NHS provider setting;
  • Extensive knowledge of directly running a large scale operational support service function e.g. cross site / multi organisational discharge related services.
