Job Details

Finegreen are currently working with a large NHS organisation, who are looking to appoint an Associate Director for Population Health and Care, this part of the NHS England UK wide Population and Health Care Programme. 

Key responsibilities will include:
  • Responsible and accountable for the development, implementation and delivery of the system wide Population Health and Care Programme for the locality;
  • Work closely with NHS and local authority partners across the borough and with wider stakeholders including community representatives, patients, service users and residents;
  • Leading the development and delivery of the Population Health and Care Programme, ensuring achievement of key strategic objectives and outcomes;
  • Working closely with the Care Partnership Strategic Board and wider stakeholders to enable and facilitate change for the benefit of residents and patients;
  • Providing senior-level expertise in the use of population health and care data across the partnership to inform the development of new models of care and the identification of opportunities for pathway and service improvement;
  • Responsibility and oversight of the population health and care programme team functionality and resources, and working with partners to make best use of system resources to achieve shared objectives;
  • Building relationships with a wide range of stakeholders; understanding their needs, challenges and priorities and facilitating collaborative working;
  • Being a role model for partnership working, system leadership and championing equality, diversity and inclusion.
The ideal candidate will have:
  • Specialist training or equivalent senior level experience in leading programmes (e.g. PRINCE, MSP) in the NHS environment;
  • Experience, expertise and understanding of performance and change management, business planning, budget setting and contracting processes, in a health and care setting;
  • Experience in the NHS strategic management / leadership;
  • Detailed knowledge and understanding of the NHS context and on-going developments.
