Interim transformation manager

Role description

The Children and Young Peoples (CYP) Transformation programme is a system wide transformation programme which incorporates the ambitions from the NHS Long Term Plan, national and regional priorities and local system priorities (including those from Place and Providers) into one cohesive work plan.

An interim Transformation Manager is required to progress the Asthma work - to support the development of an implementation of an asthma action plan and the implementation of an delivery group and workplan.

The deliverables set out in our plan for asthma were:

• Completion of self-assessment exercise against the national bundle of care.​ CYP dashboard draft to be produced. ​ Boroughs to focus on key areas of delivery with structures in place - At least 5% of CYP with diagnosed asthma offered structured education by community pharmacist (inc. inhaler technique)

• We need to develop a plan to implement the national Bundle of Care for asthma and understand local priorities around asthma We need to progress the implementation of the Digital solutions around Asthma which have been agreed

We are looking for an interim who:

• Has knowledge and experience of children and Young people work, preferably including understanding of Asthma Has key skills in project management, stakeholder management and planning Had skills to engage with CYP as part of the project Can work using own initiative but as part of a team
