Practicus are partnering with a London based NHS organisation that are urgently seeking a Head of Community Services.

PURPOSE - You will use your experience as a senior service manager to provide and lead a portfolio of adult community services implementing strategies and key initiatives to deliver integrated community based provision outside of hospital.

THE DIFFERENCE is that you will play a key role in delivering Trust's transformation and improvement plan for community services by making best practices changes and continuous clinical improvements for best outcomes for patients and their families.

THE ROLE IS BOTH INTERESTING AND CHALLENGING because you will be working with a new model to identify patients most likely to be at risk of hospital admission to remain as close to home as possible and to work with others to reduce avoidable hospital admissions.

TO DELIVER THIS we require an experienced community services senior leader with knowledge of current adult and older people's healthcare policy and understanding of the demands of delivering health care in ethnically diverse communities.

EXPERIENCE:
· Proven experience gained within the NHS at a senior management role with community or therapies background
· Ability to plan, organise and deliver services including the management of staff and budgets.
· Ability to champion strong collaboration across community service pathways and ensure the engagement of other services to maintain patient activity and flow
· Experience of leading and supporting teams to initiate and implement organisational change
· Experience of managing and maintaining effective relationships with a variety of stakeholders and able to build alliances and networks.
· A proactive professional who has responsive approach to work and a team player
